ROSE HEAD with holes

Job Description:
Looking for a rose head. It should be 3.5 inch diameter at the bottom and 5 inches in height.
I have a current model, but there are a lot of changes that i would like to see in the next rendering. However, the diameter size of the holes shouldn't be changed.
First, I would like to see it as a more realistic rose shape instead of a flattened out bottom,.
second, I would like to see THREE side holes that run up and down the rose head and allows the object being placed into them to not shoot out at an angle- which they currently are.
Lastly, Is there any way this can have a smoother aesthetic than what the model (and our current prototype) shows?
I have attached pictures to current prototype that is in hand, and the current 3d model.
I have sketched of what i would like the new rendering to look like and would be completely open to having conversations with anyone for any further clarification needed.
