Convert html to Word document with some design...

What I need done is take my current html code and convert it into a Word document for people to fill out that don't have internet access. I would still like to allow the radio buttons to be Yes/No answers and for any of the text boxes leave ample space for writing or typing information. It will require some design work on the word document something that will look nice when giving to businesses. There will be a total of 22 word documents needed and I don't for see there being over 3 pages per document.
