Build a app to count action

This standalone app that is independent of any database needs to do the following: - Capture an ID approximately 6 digits long - Capture a date of birth - Clock to run once the user starts recording, for a total duration of 5 minutes, where during this time the user is counting incidents (not related to the app) in increments of 1 up to 100 (1 +1 a simple counter), with each entry timestamped with date and time. - The counter is to be done once a day, for a total of 11 events (at different time, but no schedule is necessary), the event ID out of 11 also to be captured - Store this data in the app after submission, and send results via email or text message to a designated user - No backend system is needed We would need an approximate cost for developing this app and publishing it in the Apple and Android stores.
