We are looking for someone to create a simple quiz for our website, BreastCancerAnswers.com. We've created a mockup of exactly how we'd like the quiz to look and it's attached as a PDF. We would like each question to be on its own page and after the user chooses "True" or "False" they'll be taken to a new page where a video will play explaining the correct answer. The user can click "Next" on this page to be taken to the next question, and so on. There are five questions total, and a video to go along with each question. I will send those via email once you are chosen to work on the project. Additionally, there are two logos that need to be included on each page. See the PDF for where these should be placed.
On the close page of the quiz, we'd like to give our readers the option to share the quiz with their friends, via Facebook, Twitter, G+ or email.
