Design a Wordpress Mockup

Hi folks, Can someone create a Wordpress form with several selector made with images, like radio choice where the text is replaced by image? Example: [login to view URL] - check the home page is a giant form with images selector. The purpose is to create an ecommerce wordpress website to sell a service, with the following workflow: 1/ Client selects items in form and submit 2/ Acknowledgment order 3/ Optional: vendor can ask additional questions to the client 4/ Vendor submit a proposal to the client 5/ Optional: Client can reject or ask modification - Then new proposal from vendor 6/ If interested, purchase of the client 7/ Acknowledgement 8/ Status order
