Create simple form with ID verification via API

Simple form that will verify ID via JSON API User will enter 3 fields: 1) e-mail address 2) ID number ("Documento": "999.999.999-99") 3) Date of Birth ("DataNascimento": "25/08/2015") Bult in the request we will also have the credentials for the webservice: Email and Password Here is the JSON example: [login to view URL] All the Response (Resposta) fields will have to be stored on a database table together with the e-mail address he typed. And the outcomes will be the following: IF either the Document or the date of birth are not valid we will just show a message in RED in the form itself (like "invalid ID" or "invalid Date of Birth") IF both Document and date of birth are good all response data will be stored in a table AND the user will be directed to another html page for confirmation. IF the API service is unavailable it will create a RED alert in the form "Service unavailable. Try again later".
