Click on checkboxes in HTML, save changes to disk

We have an HTML page that works offline and lists some items. We want the end-user to review this list and choose a possible value for each item: (IGNORE, ACCEPTED, ATTENTION, FAILED). Would be great to have some color associated to these icons like (grey, green, yellow, red). End-user can change these values and on the top of the page should come a reminder saying something like "Changes were made to the report, save them?". When the end-user clicks on "Save", then he gets a JSON document called "[login to view URL]" that he (himself) will place next to the [login to view URL] of the report. Next time the report gets loaded, it will look for the data inside "[login to view URL]" and load them accordingly. On the list we need the items to be placed inside each category. If there are items without a label (for example new items), then they should be placed with status (UNKNOWN), meaning placed on top for the end-user to review. btw.
