Based on requirement given i believe it is best to go with Phonegap, html5, jquery in case you have simple 5-6 forms which will be communicating to already existing services. Also, this should not be be very costly as it should get done in 5-6 days (in case if it is simple forms) Thanks, Sanjay Jha
