A mobile database app for audits

I require a database app for conducting audits. The app will take the user through a series of questions (binary or multi-select) and include context help with photos.
Key features: Encryption, load database table from file (for example csv), load audit questions into table by importing from a csv file, import context help (with pic), export completed audit results to csv file, export to (email, Bluetooth, etc.), photo and audio fields, automated email alerts.
Based on the quality of results, there may be more work.
