ASP.net Insert Template pages

I need three asp.net insert pages constructed with cascading dropdownlists.?
The examples below? use a company example, but the actual descriptions are different.? The relationships are similar.? The actual items are geographical.
Examples:
Page 1 is to enter responses into a database table by selecting Company, then Department within the selected company, then the datetime using an Ajax Calendar control.? The page continues with 10 responses? each having? individual drop downlists to constrain the responses.? All 13 items are to be inserted into? the database table, which is already available.
Page 2 is more complex by? selecting:
1. Default? datetime with an Ajax Calendar control
2. Company?
3. Department in Company
4. Individual Datetime for Collected (override default datetime)
5. Individual Datetime for Responded (override default datetime)
6. Checkbox for each of the 3 Offices? in each department? ? (each department always has 3 Offices)
7. Each Office can have 1 to 4 response categories, so there could be up to 4 results or as? few as 1 result.? One result per category.
8. A Result field with a qualifier of Less Than <? or Greater Than? > and a response value.
9. Two Check Boxes for Type A or Type B that determine the condition of the response.
10. A Comment field
Page 3 is an administrative page to grant Company and Department privileges to a user by selecting:
1. MultiSelect Company(s)
2. MultiSelect Departments within the selected companies
3. Textbox to enter a username
All three pages perform insert into database tables.
There are old screen shots available of the prior pages this replaces, but no code.
I? have a dynamic data project with a datacontext? that will use these pages as standard asp.net pages.? I have straight insert template pages constructed without the dropdown logic, so you have something to work with.

I have a Word document that describes what needs to be done with screen prints.? The database tables have data already populated.
