ASP upload

I need an application written or an existing one modified which will allow web users to upload .zip files to a protected folder on my server, via a simple HTML form with the obvious input field that browses their drive. The form should have the following fields, which will correspond to a SQL server table with datatypes of your recommendation (I will supply you with connection info): 1. AuthorName (textfield) 2. Title (textfield) 3. Category (drop-down menu) 4. Level (drop-down menu) 5. Description (textarea) 7. Browse (file) I will also need a small test page written which displays the contents of the database, with each record containing a link to the uploaded .zip file so that users can download it. I also need the user to be able to "rank" each record; i.e., once they have downloaded the .zip and viewed its contents, they can return to the site and rank the file on a scale of 1 to 10. Unique users can only rank a file once.
