Basic ASP.NET Data Entry/Admin forms

Specs: ASP.NET 1.1 (VS2003) MS Access 2000 database to store data entry and login info Data Entry Form (viewable on web site): (Please see attached body section from html page for form field types. Don't worry about missing logos. Please make "Date" "Date/Time" and display current date/time accordingly. Currently there is a js function populating the date.) Submit (saves data to Access db) Please add javascript enabled print button to print web page. Login Form (separate link, not viewable on web site): [Username], [Password] already set-up in Access table. Credentials are verified and valid user is taken to the Admin page. Admin Page (if invalid user tries direct access, redirected to login page): Data previously entered to Access database is listed on page with following fields(columns): [Referral_ID], [Date/Time Submitted], [Adjuster], [Referring Company], [Claim#], [Reasons For Referral] Link Page: Referral_ID field links to and opens the corresponding referral page, but is not editable. This is a primary key field in the Access table. Please add javascript enabled print button to print web page. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
