CD Notifier

I want to send a cd to my prospects with a movie on it. The movie can be done in any format, but must be playable on a PC. I want the presentation to open up saying "to customize this presentation please fill in your first and last name. When the movie is played an email is automatically sent to me (the sender) telling me that the recipient (john doe) is watching the movie.? I would also like to be done if the contents on the cd is also just an audio file.
