I need an online form, that I can add to my wordpress site, that would allow visitors to reserve a number (say 1-50 or any range desires) with extra field for visitor's name and after saving it would redirect them to another page. After saving, the name should be replaced with 'taken' but the number remains visible. I as the admin should be able to see all the names and numbers. I run a baseball team and I need my players to be able to select their jersey number.

And how you suppose to keep that page private? otherwise your normal visitors also will start picking. Will you protect it with a password so only your team can go inside and do picking?
