Classifieds Order Form

Create a website order form for a classifieds. The form need to interface to mysql Data base to collect order form information, and allow export & import of data from an excel sheet. We accept two payment option Payapl and By check. We estimate the order process to take about 8 to 10 pages. The format needs to match our existing website. We have about six different options our customer can choose from. Most of the fields required by the form are straight forward. Except for one where a customer must select one or multiple categories form a list of over 900. We charge for additional category a client select. Another challenge we charge for each text line. We will provide a power point file for the ordering process flow and what is required for each page.
