AJAX/PHP form

I currently have a form that's generated via php from a mysql database. As you can see there is a 'technical reviewers' column with a plus sign next to it. I need to implement the following functionality: 1) People can add extra technical reviewer drop down menus by pressing the plus sign in an ajax type fashion up to a maximum of 5 2) Once the submit button on the form is pressed the fields that have been updated 'PAC', and 'Technical Reviewer' are entered into the database. I understand how to update mysql fields but I don't understand how to get the 5 or fewer technical reviewer fields after they've been added. Any empty fields should not trigger an update. Any fields that were previously populated but change should delete the previous record and update to the new record. The submit button should not open a new page, but should just say 'successfully saved' on the same page. This is an URGENT job.
