Date won't save to database

Hello I have a script that saves 3 out of 4 items into the database. The field that is not saving is a date field which has a calendar I can click on and choose a date. After i choose a date and hit save it doesn't save the new date so currently i have to go into the mysql database and change it there. If i change the field from the calendar to a normal field where i can just type in 2012-05-16 then it will update but it won't update when I leave the calendar on. I have this same script on many other pages of my website and they all work great so there will plenty of example you can view but in my limited knowledge of php I can't figure out why it won't work like the others. I've attached an image of what the calendar and page looks like and i've attached an image of the code where the problem is. I wouldn't imagine this will take a good php programmer more than a few minutes to fix. Thanks
