Contest Voting Page Script Installed / CSS written

I need a voting page built. I have attached a JPG of what I want the page to look like. I already have the actual page built, I just need the css written for the entries and the voting script installed. The voting must work this way: a person can vote for 1 entry only, after hitting vote the page will refresh and the results will show under each entry. They can hit vote again upon returning to the page, but it will not count (one vote per IP only will count). I already have the MP3 flash player installed on the site.
