Basic PHP authentication page

I have an HTML website built in Frontpage. I need to create a basic authentication page using PHP that uses one user ID and one password for all users (I can define the username and password later), without the use of a database if possible. I require the code for a basic authentication form that has a field called "User Name" and another "Password", with a button "Log In", when clicked, directs users to a file called "[login to view URL]". Since the code will be placed in an existing Frontpage file called "[login to view URL]", please supply documentation on where code should be placed.
