Build a page which can upload image and video visually

Similar to a CMS I want a page which allows the user to upload images, videos and create text fields.
The page will show a blank area for media to be created. User can either click an add media button or drag and drop media into the area. When the media is added, it will show a preview of the media. Beside that it will have options to save the media under a descriptive name and then upload the media. When the upload button is clicked, the media will start to upload, but the rest of the page will still be functional.
After the upload is complete the background will change color and the upload button will become a tag button. The tag button has no functionality yet.
Upload formats will need to include ogg, wmv, mp4, mov, jpg, bmp, gif, png with a size limit of 50mb.
THe UI should look good, ie when an item is dragged over the drop area a plus symbol appears etc.
Clarifications required.
1. Does this a standalone application or to be integrated into some other website. If this need to be integrated somewhere, then what technology or opensource used to develop the website ?
2. What is the purpose of the uploads ?
3. The uploaded files can be saved in the serve, however do we need to track the uploads by the user ? (The upload data to be saved in database ?)
4. After the upload is done. Then what is next ?
5. Before showing this form for upload ... Where to arrive at this form ?
6. Can you provide us more details about what the site is about and what you intended to do with the uploads
