Change to contact form on Concrete5 - repost

I have a small website in the PHP CMS Concrete5. There is a contact form on the website that generates an email to the admin. I need the format of this email changing. Its only a small job that should not even take an hour, but you need to know Concrete5 really well to know where to change the email format. I will supply the existing format, new format and all webfiles for you to look at.
Note: This is a repost as the last freelancer could not complete the work - it would be ideal if you know Concrete5 as you will need to create a new array in the php handler to get the fields and reformat in the way that I want.
