Convert paper and excel based psychological tests to LimeSurvey

For a mental health center we need someone to convert paper based and spreadsheet based scoring of psychological tests to LimeSurvey. The project will consist of the conversion of files like the attached .xls to LimeSurvey. A number of tests do not exist in spreadsheet form yet, and will need to be converted to LimeSurvey based on the paper based forms. We prefer to work with a Dutch native speaker, since our forms are mostly in Dutch. In your PM please explain whether you only bid for conversion of the attached file; or whether you bid for a larger number. If you can provide us with an example (create the BSI score sheet in LimeSurvey; export from LimeSurvey and supply that file to us so we can test it locally) - then we can do business very quickly. We will pick you as the bid winner as soon as we are satisfied that you can do the job and your price is reasonable. We are looking for a long term cooperation; both for this and for other projects.
