Aweber Script Fix

Hello, I have a script installed on a webpage that allows me to override the 80 character limit for a text field on an aweber form. What it does is combine multiple hidden fields and when I export the data to a spreadsheet I use the concatenate function to string the data back together. My problem is that: 1) I need more hidden input fields so that I can capture more characters. 2) For some reason the form isn't passing data from 2 of the fields sometimes (phone # & a check box) I also need a couple of small interface tweaks. I just need the name and e-mail address on the same line and the Male/Female radio button and Age field on the same line. A couple small changes in the wording and a different button graphic.
