Convert current custom Wordpress PHP code to Wordpress Plugin

I need my php code converted into a Wordpress Plug-in. The code is already working and running in the Wordpress Theme [url removed, login to view] file.

I need to have this PHP code, listed here [url removed, login to view], in the form of a Wordpress plugin that can be enabled and disabled. Currently I have to copy and paste it into the [url removed, login to view] file.

I want to have 2 options available in the plug-in settings page that I can retrieve the value of in the code. This would be a label, a text field and a checkbox.

It should also fulfill any requirements necessary (if any) to be listed in the Wordpress plugins listings.
