Build 1-3 step installation process for existing script (simple)

I have an existing script that resides on my server. Originally for personal use only I now need an installation process for new users to install on their own servers. Ideally person would upload the script and then run the install that prompts for to information: username, password, and any other info required like the mysql database that will be used. Goal is simple setup for the user so they are not required to manually play with phpmyadmin or anything else to get this script installed and setup.
