convert psd to css\html

hello everyone, i'm working on a psd template and should have it ready for you soon. i'm looking for a person who can take the psd file and convert it to css\html. the template is very, very simple and it contains only one page and one form. actually two pages but they are both the same. the only difference is that one page has the form which should be done in php as well. the form will only have 4..5 fields. the right person should be able to get this done in a few hours. following things are a must: - you must send me a few samples or links to working samples - the template should look the same in firefox,internet explorer and chrome - you must sign a NDA - you must have good feedback score - the price should be right(should not exceed $50)...this shouldn't take you long at all and i can give you more info before you commit and give me the exact price. - you should be able to start working on this next week (friday) and get it done in three, four days. thank you and good luck
