Convert a Template to a Website

I would like someone to take my template which is live at [login to view URL] and create several sub-pages (convert template to a full web site). I want to to appear highly professional with forms that look like the ones on [login to view URL] - please have excellent CSS knowledge to create the following: 1- I want to change the bomb icon next to "search by price" to a dollar sign 2- a professional contact us page that demonstrates all of the form fields including text, password, check box, radio button, listbox, message box, and any others I might have forgotten. This is basically just setting up the styles to make the form fields look professional like thos on [login to view URL] 3- a professional list of diseases (see the home page for example how this area should look "pain and stress" "respiratory and digestive" this page will list 100 different diseases so please try to condense the information into a smaller area than that of the home page.... you just need to provide a workable format that I can customize. 4- a page that lists several acupuncturists including a "mini profile" that lists their name, neighborhood, years in practice, and their phone number. just provide a workable format that I can customize later. 5- a page that lists acupuncturist's COMPLETE profile including their name with their photo, and some information about them including number of years practice, their degrees, schools they went to, their neighborhood, address, city, state, zip, and phone as well as their payment types accepted, their insurance types accepted, their treatment styles, and the diseases that they treat most often. Also has links to their yelp reviews and their google plus pages. So basically its just filling out these pages using the existing home page as the template and making it looks highly professional too.
