I am looking to create a simple forms based website which stores the data captured and provides PDF copies of submitted forms in a central console.
This will be a simple forms HTML site used by Smartphone, ipad and computer browser users. Requires the following form capture functions:
• Enter client name
• Client address – using google maps lookup, can also use local gps location
• Basic form options using checkbox, dropdown list.
• Use reference data from a CSV file or similar – the file will store employee names and data to pre-populate form fields.
• Login using Google (openID)
Form submissions are emailed to the admin email account.
These can be centrally referenced and the web console and printed out as PDFs where needed.
PDF will include existing company logo and contact details.
I don’t mind what product is used to create this, as long as the requirements are fulfilled.
If its an out of the box forms service available on the internet, or a custom made site with a database or similar.
This site will be used as a prototype with potential for further similar sites to be built.

I do not need any site content, this is purely a forms site used by employees.

It is important to have the form submissions stored where they can be viewed again either by the submitter or a central admin user.
