Database lookup app (IPhone)

We are looking to create an iPad/iPhone application that does the following: 1. Scan a bar-code of a product (using a camera as bar code reader) 2: Application then will look up the bar-code of a product on a mysql database on the web 3: Display results from the mysql* on a single page: * displayed results 1. Company name/Brand 2. Barcode number 3. Last date updated in the database 4. Product name 5. Issuing authority (up to 5 different ones) 6. Product weight or type 7. Picture if available as thumbnail 8. Large image/stamp stating YES - NO - UNKNOWN - UNDER REVIEW 9. Submitted by User / Authorized authority 4: If the product does not exist in the database and the application has scanned the bar-code, the application should ask the user if they wish to submit the Product for review, then the user needs to enter the following details: 1: email address* (must) 2: a picture of the product* (must) 3: Company name/Brand (optional - will be added and approved later by internal staff) 4: Product name (optional) 5: Issuing authority (up to 5 different ones)* (must) 6: Product weight or type (optional) 7: Bar-code Number (already captured when user scanned the product) The application needs to submit to the database all the above information. 5. There needs to be a contact page with telephone numbers and a form to send emails to us. 6. Very important: the application must have the ability to sync the database to SQLite on the iPad/iPhone for offline usage. 7. Report a false / No longer relevant product page/button. 8. Application will open immediately with camera for scanning option. 9. Product will update automatically on launch with the master Database (if user is online) 10. All buttons should be large, clear and pretty :) 11. After the information has been submitted, there should be a pop up window notifying the user about it: "Thank you for submitting the product. We will review it and it will soon appear in our database." 12 the application must update products even if offline once the user is online then it will sync their submitions More minor details on the project will be given to the developer, such as product name and emails, telephone numbers etc once the project has been approved. The back end of the database will be given such as username and passwords once the project has been approved.
