Custom survey development

Hi I need help putting together a survey. The purpose of the survey is to choose between groups of images. IT will all be image based. I basically have 100 designs for mats. These designs are broken up into smaller groups of between 5-10 images. Firstly, I want the users to be able to choose the images they like from each group - simple checkbox. Secondly, any images they clicked on the checkbox that they liked I want these to be populated into another survey and they choose maximum 15 images they like from the total group I can supply all images the presentation of the survey is key. The survey will be for around 100 people and I want to collect the results Thanks
