build an excel form

I need to design a EXCEL form that will export data to three places. The project has an excel "form" that the front end user will select dates, select from drop down, and type text. Some of the boxes here will auto populate based on user's choices. After the user is finished, he will click a submit button that sends the data to 1) a database (that collects all the entries in one place) 2) as a pdf (locked without editing) to the user's email. 3) to the administrator's email as a locked pdf that contains other fields referenced to the data mapping I will provide you. Lastly, it is important for me to have the ability to edit the excel you create to add to the table. 1) the information in the form is highly sensitive so I will have to change the text 2) fields will need to be edited from time to time as needed to be updated.
