Link website form to Mailchimp form

Job Description:
I need someone to link my website form to my mailchimp account. The Signup form is already active in mailchimp and online. Mailchimp is already linked to the website with another form (newsletter in footer) so the task should be very quick and simple.
Required.
1. Link this form (attached) to mailchimp so all data is automatically added to mailchimp when a customer submits the form on the website. [login to view URL]
2. I then need the ability to turn on/off new registrations so new applications can not be submitted via the website when out of season.
3.. Remove this other from ("sign up for our email updates) from the footer of each page of my website.
