Medical Data Dashboard

I can develop a web application in PHP as per description given in the attached MS Word file. Please send me the details of the API using which the data will be transferred from the mobile device to the web application. Once I will establish communication with the API from PHP script then I will proceed further and make live this project.

i have my own strategy to work in java. i am using below technology. Java and advance java jsp,jstl,hibernate,json tools : myeclipse for spring. mysql workbench,maven dependency i have my own jar file made to work in java to save time.

I have worked in Infosys a top IT services company for more than 10 years and now i have started on my own. Why not use Grafana for the Dashboard system rather than build one from scratch? I can setup and configure a web based Grafana system for you.
