tournament registration script

am looking for a script that our tournament can use to register players to the site but also give them a choice of 3 games and 2 teams to enlist for.

if 2 games is to many just selection of the teams would be good enough.

something like:

username: ----------

Password: ----------

Confirm Password: ----------

Email address: -----------

Select Game: ---------

Select Side: allies ------ : Axis --------

Submit.

It would then register them to the site and assign them to a usergroup of the selected Team/Game.

I would also require the ability to change members around if needed and edit any of their preferences.

Also a Team/Game roster. would be a nice bonus but not essential.
