Output User Input to PDF

I need to add an automator to my website that will allow a user to input there data:
IE: Name, address, phone number, Driver license and other details as I need.
once the user is finished they click on submit and the information they entered will be inserted in the precompiled text I have already created giving the user a finished document in PDF.
I also need to have this only allow a set number of times the user can make these before it locks them out, lets say 30 times
