Brief A small clinic with about 200 patients needs a database driven system with very simple user interface to input, edit, view and print data. There are 2 main users of the system: The Administrator, and the Doctor. Functions - The system is to be designed in such a way that the Admin, once logged in, can select “ADD PATIENT”. This function adds all patients to the database by filling out patient details to a simple form that includes dropdown lists, checkboxes, upload images (x-rays, scanned documents) etc. - The Admin is to be able to make appointments for these patients by filling out an “APPOINTMENT FORM” (date and time, purpose of visit). - All appointments should be able to be viewed by both Admin and Doctor. - For each appointment, the Doctor can add PRESCRIPTION, INSTRUCTIONS, and DOCTORS NOTES. After doctor has added this info, Admin can add SUMMARY paragraph. This information is to be saved under the patient’s history so that it is visible to the doctor the next time the patient comes in for an appointment. - The Doctor’s interface is to be very simple- Once logged in to system, Doctor must have options to VIEW CALENDAR, VIEW PATIENT RECORDS, VIEW PATIENT HISTORY. Printing of reports is necessary. The Doctor’s interface must be very clean and tidy, navigation made easy, future appointments easy to navigate.
