Need a simple hybrid application done

Requirement:
User downloads the app.
On the first screen we have an agreement.
User accepts it by tapping on a button.
User can then select a name from drop down(we have a wordpress website. we need to pull the users from there using api).
User can then take a picture or capture a video.
User can then write a testimonial(or a short texts) and then submits the form.
The picture/video and texts should be then mailed to two email IDs. One is admin email, which is always same. And the other one is the user selected from drop down.
Please let me know if you need any clarifications. We are looking for a team or individual developer who can provide best quality and maintain the commitment.
