I wanna add affiliation on a wordpress theme

Our platform is a doctor listing platform, doctors will pay us so they can be listed on our website. The payment methode will be cash (we have 4 packs, bronze, silver, gold and diamond), so no integration is needed, every doctor who signup will stay pending untill we manually approve it, and give it the proper pack he payed for. Every doctor will have a section on the menu or his back office called "contact": that section contains every doctor or hospital who signed in through his affiliate link. So every doctor can see his affiliates and the pack they took (bronze silver etc), their affiliation comission will be given to them by cash, so once more no integration needed.
