Web Forms javascript html css php ajax jquery jsgrid

Hi, I am looking for someone to help with web forms using javascript, html, css, php etc. I have a localhost environment with the form working but need little tweaks and requests changes from time to time. Currently the form is working. reading and writing to the database but I wanted to add a multiple selector option to one of the dropdowns. This is giveing me some trouble. I want to be able to mutliselect more than one option. I have downloaded and am using some jquery plugins and datepicker plugin for one of the dropdowns. Another tweak I want to make is that when someone selects yes in one of the dropdowns I want this to present two new dialog boxes where the user can enter more information. This is where I am at so far in the project. So two problems. 1. Get the multipleselector working. Where it can add multiple entrys to the database. Currently it works for editing but not on adding. so this might be only a small problem. 2. Is getting the 'if yes present two new dialog boxes to enter information working. I havent got this near working yet. If you could first clarify how you would work on the project I can then award it. 1 - Do I send you all the files? (Quite a lot of files) and if so do you need everything including the sql database? I am using jsgrid, datapicker,multiselector and other little plugins so would I need to package everything up and send it to you? 2 - Would a remote session be easier? I am looking for someone who is reliable and can be contacted again in the future. If you can help and do a good job then 100% more little tweaks will be needed in the future. Thanks
