Basic Flask document generation

I'd like a Flask app that takes user input from a form and generates a Word document using python-docx that includes the data provided by the user. The system should not store the document, but stream it directly to the user (see [login to view URL]). Input fields are: (i) file upload for signature; (ii) text input for name; (iii) select input for state; (iv) checkbox for whether to include a certain paragraph.
