Document Report Template

I'm needing someone who can translate a design aesthetically, and add some interactive features to a word template that will generate a report once populated via input (from VSTO scripting) the dialog boxes will walk the user through populating the document until it's ready to export/save/print ... I understand it's rare to find someone who can do good design and code well enough to nail this, but ... thought I'd try. I've attached a pdf to get an idea of the aesthetic, it's important that this layout is respected, with _some_ room for artistic freedom, each element of the report will probably need a discussion, refining out shared understanding of what the outcome needs to be, then deliver that piece of the cake. The immediate features are: easily changeable front cover graphic (new image on the front page per report), a UI for inputting populating some of the variables that persist throughout the report (client name, engagement type, project name etc) some cross referencing of overall issues (echo those stats out elsewhere in the document) and some strict document style (I'll provide the font) - this will probably be quite an interactive job, I want to use the report for myself, but possibly share it for others to use if I decide to share it on GitHub
