Templet for productivity and an achievement report + 3 logos for:
1- Radiology information system
2- Laboratory Information System
3- Hospital Ancillary units Technology

By template I mean: graphical design template that can be used every month and I can change it content as I wish.
I will provide you a sample for the report and explanation for logos

Important Note: any design either for the logos or for the template should reflect the following:

1- Information Technology
2- healthcare
3- National guard health affairs Saudi Arabia. http://ngha.med.sa/English/Pages/default.aspx
4- attachment is a monthly report content example. ( present the tables in ore professional way)

the more smart design there is, the more bonus you get
