edit a picture and add some text / icons

please see the attached picture "original"
I need this to be edited - i have made a mock up example of this but i did this very quickly and it is just to show the type of thing i want .
you need to add the three icons but i want these in circles not squares and the font for the text next to these can you use "send in him"
I want the words "rehydration & recovery" written in big bubble writting and i do not want this straight i want it the same angle at the red line
and finally like i have done in the mock up you must make this a square image
