Convert Photoshop File/Images Into HTML/CSS - Modify Existing Website

We have an existing website that we're looking to have the form modified on in order to ask several survey questions. Our existing website is www.ConsumerDebtAid.ca. Please see the attached files for the format we're looking to convert our form to. We'd like the form to function similar to www.ConsumerDebtAid.co.uk.
The images required to do so are available in .PSD format. We can slice the images and export them for you if necessary, though we'd prefer to find someone with the necessary Photoshop skills. We can also do the php required to integrate the form into our website ourselves, though it would be great to find someone with that skillset as well.
We have many additional projects we'll need done in the near future and are looking for someone who we can make part of our team and send business to on an ongoing basis. Specifically, we're looking for excellent communication, fast turnaround times and superb work quality.
