Design ITouch/IPad/IPhone app that can be exported into an excel spreadsheet

Opening Screen --> Master list (to choose location)
Screen 2 --> Gives Name; address; phone #; contact and e-mail
Screen 3 --> room # list (has total room list for specific hotel and another box to enter individual room #) need # key pad and alpha key pad. Once individual room # is keyed in will take you to the next screen. This screen should also have on the bottom Rooms searched & rooms left (these are the two items that will need to be exported to an excel spreadsheet along with the explanations)
Screen 4 --> Confirmed; Non-Confirmed Alert; Spotting/Castings; No Alert (No Alert will take you back to Screen 2) The other alerts will take you to
Screen 5 --> Bed; Headboard; Sofa; Chair; Pictures; Baseboards; other; Done (each will have a box next to it to check off) Done will take you back to screen 3
