Custom .Net Application - Build prototype

I need a .net developer to create a custom application that is web based. The App will be prototype of the larger application. My goal right now is to build a small part of the application to show the interface to potential investors. I have the basic requirements that will be needed.
This could turn into a longer term project once funding is secured for the full application. Right now I am doing this on a budget and understand it may take a few months to build the prototype. I will own all the code generated for this prototype.
The main goal of the application will be mostly for tracking dates, approvals, and email notifications.
The DB should MS SQL.
The developer will help design and create the main db tables used to drive the forms.
For the prototye we are looking to capture:
1. Main form for contact information
2. child form for product details
3. Move through four phases: Initiation, Review, Approval, and Complete.
Emails must be sent at every step to a configurable group of users.
4. Cost from the product details must be displayed in the parent form.
5. Form to drive menu values.
I am looking to spend $100-200 to start and then set a monthly budget going forward based on you estimate.
I need to know how much work can be done with this budget and how long.

The development code must be included as part of the project and included with any installation package. I will own all code.
