Extremely simple project. Need a simple script that allows people to enter their name, email address, and affiliate ID from [url removed, login to view] into a form on the page and the affiliate ID is populated into various places on the web page that have xxxxx in them and the name, email address and clickbank ID can be sent to me via the Autoresponse Plus autoresponder software located at [url removed, login to view]
Here's an example of a webpage that uses this technique...

Notice the blue box where the visitor enters their email address, name and Clickbank ID and when they click the submit button, the xxxxxx spots on the page are filled in with the affiliate ID entered into the form. I also need the email address, name and affiliate ID entered into the form to be sent to my Autoreponse Plus autoresponder.
This is an extremely inexpensive and easy task for someone who knows what they are doing.
