Convert Joomla 1.0 Template to Joomla 1.5

I want to update my father in laws Joomla site to 1.5.X from 1.0.x I have a template design that was used on his 1.0.x site and must be used, but currently doesn't work in 1.5, due to the code changes implemented in the Joomla upgrade. I need the template updated with the 1.5 code so it works exactly the same as it did on the 1.0.x site.
Looking for someone experienced in taking a Joomla 1.0 template and updating the code, particularly in the "index.php" file so that it works flawlessly in Joomla 1.5. Attached is the existing "index.php" file to look at so you can see the level of effort. We are assuming a pretty low level of effort for someone experienced with this kind of update
