Create a monthly and yearly customized calendar: Tooltips, MySQL, Php form

Create two calendars: monthly view and yearly view (daily view not needed) for a musician's website.

Calendar event details will be entered by admin via a PHP form and sent to a database via MySQL (please, mysqli or PDO only).

Monthly calendar:

Event title will display on the calendar. When the date is hovered, a tooltip pops up with the event title, image (optional), date, time, link to map, and link to ticket information. Should be responsive, and when at around 40em, the calendar disappears and only a list of events with dates, times, and links appears (no image).

Yearly calendar:

2014 calendar with twelve months visible, formatted in 4 rows of 3 for large screen, and responsive (6 rows of 2 for tablet, then 12 rows of 1 for handheld).

No event information will display on the calendar, only a highlighted date. Tooltips pop up on hover, with the same information as the monthly calendar.

PHP form:

Required fields are event title, date, times (with an option to enter more that one time, as there may be two performances in a single day), and map url.

Optional fields are image upload (with resize function, as tooltip image will only be about 200px X 200px), venue URL, and ticket URL

I will add my own validation after the project is finalized, so only the simplest validation (for testing) is required here.

InnoDB, utf-8 general-ci encoding

CSS:

I will attach a photo of what we're looking for regarding basic styling requirements. If you have any great design ideas, you're more that welcome to contribute. For this project, we're only concerned about the calendar styling (not the rest of the page).
