I require a dB Import Tool consisting of;
online screen / page with parameter fields to import csv file (file attached), data directly into a table called "data01" in a online MySql dB, "importer". The user must be prompted in required fields for the path, username, password, dB name and table name and then final [login to view URL] final password must be collected from a php file called "[login to view URL]".
Finally the page must have a submit button to submit the csv data into the table. (Service Providers must be able to show an onlone working example) See the attcahed file for the table "data01" lay out.
